Solution to Address the Social Drivers of Health in a 5-County Region

We want to create an environment where the essence is the people, focusing on unconditional acceptance and non-judgment so that each individual can shine as the best version of themselves.”
— John R. Reeves III, MHA – Health Administrator, TRHC
BRENTWOOD, TN, USA, July 17, 2024 /EINPresswire.com/ -- The Confederated Tribes of Coos, Lower Umpqua and Siuslaw Indians (CTCLUSI) have chosen eTransX’s Wellbeing Care Community Solution as a foundational platform to address both healthcare and wellbeing for underserved tribal and non-tribal populations in a multi-county region centered around Coos Bay Oregon.

This announcement is part of a transformational effort to improve health and well-being outcomes that has included the opening of the new Three Rivers Health Center (TRHC) facility in Coos Bay, Oregon on June 1st, 2024. This location brings much-needed primary care, behavioral health, and pharmacy services to the area. CEO Lee Ann Wander shared, “This is a lot more than just a health clinic; this was a promise we made to the Tribal membership when I came in. To lead with a good heart; to listen to the tribal membership and what their needs were. Healthcare was at the top of the list, and this is a promise fulfilled.”

John R. Reeves III, the Healthcare Administrator for CTCLUSI, states “We want to create an environment where the essence is the people, focusing on unconditional acceptance and non-judgment so that each individual can shine as the best version of themselves.”

Dr. Pratip Nag, CCO/CMIO, a leader in providing holistic care, shares his view on meeting whole-person needs beyond the traditional interactions “What we’re trying to do is address what we know – that while 20% of a human’s life may be determined by medical actions, 80% of a human’s life is determined by everything else happening in their lives. Our goal is to reach beyond the clinic walls, supporting the other 80% through a holistic approach that integrates the best of modern medicine, traditional healing practices, and addressing the Social Drivers of Health”

To accomplish this vision of addressing the Social Drivers of Health (SDOH) a solution was needed to connect medical care and behavioral health with community based service providers who can help with resources related to the SDOH needs of the patient. The eTransX Wellbeing Care Community solution was chosen because it not only met those core requirements but also provided a way to connect directly with patients to share updates and educational material, perform assessments, set appointments and alerts, as well as communicate directly with providers in the community. This allows stakeholders to access real-time information to track patient progress, communicate securely, and receive alerts if a patient falls out of compliance due to any barrier. The system is fully HIPAA compliant and employs role-based access protocols to protect client information.

eTransX will also employ their vast experience in healthcare data interoperability by integrating directly with the NextGen EMR system to manage demographics, referral requests, and assessments. This ability to connect on an operational level with healthcare is a true game-changer for transformative community wellbeing initiatives. Additional CBOs will then be onboarded into the consortium, with eTransX providing necessary systems training.

eTransX is excited to provide this comprehensive community-based platform that will help CTCLUSI scale their efforts to improve the quality of life for the people of the region through improved operational efficiency, improved communications, and improved data collection capabilities.
